The Shrine of St. Maria dei Miracoli is composed of three superimposed churches. The lower level, with three aisles without transept, coincides with the ancient Basilian crypt of Santa Margherita (IX-X century), where in 1576 she was found an image of the Madonna and Child, late Byzantine style, once revered by faithful as "Our Lady of Miracles." The altar and the temple of marble, silver and gold, in front of the image of Our Lady, dating back to 1886, and were donated to Francis de Bourbon, son of Ferdinand II. Along the aisles have tombs of bishops and prelates. The intermediate church is known as church of the Crucifix, and it dates from the seventeenth century. The altar and the walls are painted with figures of prophets, and scenes from the Passion attributed to the workshop of Marco Pino (late XVI cent.) There is also an organ located in the nineteenth century. The upper church, elevated to minor basilica by Pope St. Pius X, has three aisles: the nave has a wooden octagonal coffered ceiling with gilded rosettes and heads of angels on a blue background (1633), is enriched by three panels of the Madonna of Miracles, St. Benedict, St. Scholastica. Behind the altar there is a carved and gilded wooden choir (of 1644) and an organ of the eighteenth century. On the high altar in polychrome marbles and built by Marino Palmieri (of 1773-74), are depicts the three theological virtues. According to tradition, the upper church was built by Cosimo Fanzago, active in Naples in the first half of the seventeenth century.
